Make sure you have the following documents prior to registering your student.

ALL forms are required:

• Birth certificate

• Immunization record

• Social security number

• Utility bill

• Withdrawal form or last report card (if coming from another school)

• Guardian/Parent's drivers license

• Mortgage statement or lease with student's and guardian/parent's names
