Attention New Hires
New employees to the district must enroll in insurance within 31 days of his/her hire date. Please drop by the Benefits Office to pick up your new hire benefits packet.
If you do not want to participate in the district's health insurance, TRS/BCBS requires that you complete a TRS declination certification form.

Health Insurance
MISD participates in the State health plan with TRS ActiveCare which is administered by Blue Cross and Blue Shield of Texas (BCBS) and Medco. The district contributes $1800 and $2664 (depending on which plan you are enrolled in), and the State contributes $900 per year to your health insurance.

For Mesquite ISD health plan participants, we are excited to promote the Mesquite Employee Health Center (MEHC) at 300 West Kearney in Mesquite for all your primary physician care and prescription needs. For more information, visit the MEHC website.
On January 1, 2011, the Mesquite ISD began using TRS ActiveCare as our medical plan provider. You may call Blue Cross Blue Shield of Texas (BCBS) Customer Service for information about the TRS ActiveCare plans: 1-866-355-5999.

Application to Split Premium:
Use this form ONLY if you and your spouse work for different school districts and you qualify to split the family premium. Do not use this form if you and your spouse both work for MISD.
Dependent Child's Statement of Disability:
Use this form if you will be requesting TRS-ActiveCare medical coverage for a disabled child over age 25.
Transitional Care Request Form:
Use this form if you will be using a non-network doctor under TRS-ActiveCare. The criteria to qualify for transitional care are explained in the TRS Benefits Booklet on page 38.
BCBS Claim Form:
Use this form to file a claim for reimbursement. The claim form is used to pay insured/subscriber. Important: Do not file this form if your Provider of Service is submitting these charges to Blue Cross and Blue Shield of Texas.
BCBS HIPAA Authorization Form
TRS ActiveCare MISD Declination Form:
Use this form to decline health insurance for you and/or your legal spouse and/or legal dependents. This form should be completed as a NEW HIRE and/or during every ANNUAL ENROLLMENT if you are declining the health insurance.
Other Benefits/Insurance Programs
AUTO/HOME INSURANCE is offered through Liberty Mutual Advantage. Mesquite ISD employees are eligible for discounts on premiums through this program and they are payroll deductable. DENTAL INSURANCE You may use your own dentist or use a dentist from the Voluntary Preferred Dentist Program (PDP) with the MetLife dental plan. EMPLOYEE ASSISTANCE PROGRAM (EAP) is a free service available for all Mesquite ISD employees and their legal dependents. Call MHNet at 1-800-492-4357 for family/marital concerns, job related issues, legal/financial resources, depression/anxiety, or alcohol/substance abuse. FLEXIBLE SPENDING ACCOUNTS FOR MEDICAL EXPENSES AND/OR DEPENDENT CARE can be set up for payroll deduction before taxes. Total Administrative Services Corporation (TASC) administers this program. For more information, visit www.tasconline.com or call 1-800-422-4661.
GAP INSURANCE is available through American Fidelity/First Financial Group. The coverage is limited and coordinates with most group health coverage to provide reimbursement to you for medical expenses incurred. The number is 1-800-654-8489. For more information, visit www.afadvantage.com.

RETIREMENT PLANS Notice of voluntary tax-deferred retirement accounts to all MISD employees: The IRS allows school district employees the option of opening a voluntary tax-deferred retirement account and requires that Mesquite ISD inform you of this available option. Employees need to seek the advice of a financial advisor or other agent in regards to opening a tax deferred retirement account. Salary Reduction agreements for payroll deductions can be found on National Plan Administrator's website at www.natlplan.com. Click on "For Employees." Select your State and Employer under "Tax Sheltered Accounts (TSA)."

CANCELLATION OF INSURANCE Cancellation forms need to be filled out if an employee chooses to cancel his/her insurance. Telephone or email requests for cancellations cannot be honored; only cancellation forms with your signature and date can be honored. Due to government regulations, certain requirements must be met before canceling any insurance.
