Annual Benefits Enrollment

Benefits annual enrollment for 2017-2018

Enrollment opportunity will be 7/1/17 to 8/22/17

All insurance benefits roll-over except for TASC unreimbursed medical and dependent care accounts.  See CHANGES link to review changes for the new plan year.

Everyone needs to go online to the benefits enrollment system (www.in-roll.com) to:

1.     Enroll in MEA membership (Mesquite Education Association).  Membership and beneficiary information for the employer paid basic life and ad&d insurance will now be maintained online in www.in-roll.com.  All active employees who are a member of MEA and work at least 18.75 hours/week are eligible for the $10,000 employer paid policy (subs and retirees are not eligible).

2.    Decline the health insurance if you are NOT enrolling in the MISD health insurance at this time (click on the “waive coverage option” in the Medical Enrollment Section)

a.     Log into www.in-roll.com

b.     Click on the Medical link on the left side of the screen under the Status Bar

g.     Select [WAIVE COVERAGE]

h.     Click Submit and Continue button

i.      ON THE NEXT PAGE, YOU WILL DECLINE YOUR SPOUSE and/or any eligible children ………….drop down options to say why you are declining spouse and/or kids (i.e. Other coverage)

j.      Click Submit and Continue button

k.     Final Step……….click CONFIRMATION STATEMENT on left side of screen under the status bar; review for accuracy.

l.      If you look in your FILE CABINET (click on “File Cabinet” at top of screen), you will see a TRS declination form in your file cabinet, which meets TRS and ACA regulations. This is for your protection. If your spouse involuntarily loses coverage………….say, he loses his job, this declination is his ticket to get onto the health plan within 31 days if you come into the Benefits Office within 31 days to complete enrollment paperwork to add him. TRS requires that he have a declination on file in order to add him.

 

Do you want to:

  1. Elect Dependent Care and/or Unreimbursed Medical Flexible Spending Accounts (administered by TASC)? This benefit requires re-enrollment each year. If you are a current flex plan participant this benefit election does not automatically roll-over.
  2. Add, change, or cancel a benefit?
  3. Add or delete a covered dependent (spouse/child)? (see Dependent Eligibility guidelines for more information)
    1. Do you have your children enrolled in Dependent Life Insurance?  If yes, are your children over age 26?  If yes, your children are not eligible for this coverage anymore (unless approved handicap status).  Make sure you cancel this benefit in INROLL if you no longer need it.
    2. If your child is handicap status, complete continuation of coverage paperwork BEFORE the child turns age 26.

If yes, then log into www.in-roll.com and make changes.  Be sure to click on CONFIRMATION STATEMENT when your enrollment is complete to verify your elections for accuracy.

Benefits Enrollment for New Hires

You must complete your benefits/insurance enrollment within 31 days of your hire date. If you fail to do so, you will have no coverage. If you do not enroll within your 31-day election period, the next available enrollment will be the Annual Enrollment period (unless you have a life changing event during the plan year – i.e. birth, marriage, divorce; and complete paperwork within 31 days – see MISD Employee Benefits Booklet for more info).

Benefits enrollment for new hires is online at www.in-roll.com. You cannot begin online enrollment until your information is entered in the MISD payroll system. If you are unable to log into the enrollment system a week after your hire date (the first day you begin working), please contact the Benefits Department for assistance. You do not want to miss your 31 day window to enroll in insurance. If you enroll for coverage within 31 days, insurance is effective the first of the month following your actively-at-work date. NOTE:  If you enroll in insurance after a payroll cycle has occurred, we will have to double your insurance premium(s) on your next paycheck. It is in your best interest to enroll in insurance as soon as possible so you can have access to your insurance, as it is effective on the 1st day of the month following your hire date.

The TRS Enrollment Guide provides an overview of TRS-ActiveCare health plan benefits. Please note that the premiums listed in the TRS Guide do not reflect the employer contribution for the health insurance; however, the MISD Employee Benefits Booklet does.

New Hire Presentation


Benefits Enrollment Throughout the Year

Enrollment (adding insurance, changes to insurance and/or cancelling insurance) throughout the year can be done within 31 days of a Section 125 qualified event status change. The employee must complete paperwork in the Benefits Office within 31 days of the qualified event. A phone call, telephone message or email notification is not enrollment in the plan. A qualified status change may be any of the following:

Birth or adoption of a child

  • It is not necessary to wait for the Social Security number to enroll your newborn; however, the Benefits Office will need this information as soon as it is available.
  • Please provide the Benefits Office with a copy of the Verification of Birth Facts (from the hospital).
  • TRS-ActiveCare automatically provides health coverage for a newborn child of a covered employee for the first 31 days after the date of birth, but this coverage ends unless the newborn is added to the employee’s health coverage within in the deadline.
  • For the newborn, the effective date of coverage with TRS-ActiveCare (health insurance) will be the newborn’s date of birth.The effective date of other coverage (i.e. life insurance, dental, vision, etc.) will be the first of the month following the date the paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).
  • You may cancel your insurance within 31 days of birth of the baby.  The effective date will be the first of the month following the date paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).  Please provide Verification of Birth Facts.

Marriage

  • Please provide the Benefits Office with a copy of the Marriage License.  The effective date of coverage with TRS-ActiveCare will be the first of the following month after the marriage date.  The effective date of other coverage (i.e. life insurance, dental, vision, etc.) will be the first of the month following the date the paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).

  • You may cancel your insurance within 31 days of marriage.  The effective date will be the first of the month following the date paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).  Please provide a copy of your Marriage License.

Divorce (if the divorce results in a loss of other coverage)

  • Please provide the Benefits Office with a copy of the Certificate of Creditable Coverage that shows each plan participant that lost health coverage, the effective and end date of coverage.  The effective date of coverage with TRS-ActiveCare (health insurance) will be the first of the following month after the loss of coverage date.  The effective date of other coverage (i.e. life insurance, dental, vision, etc.) will be the first of the month following the date the paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).
  • You may cancel your insurance within 31 days of divorce.  The effective date will be the first of the month following the date paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).  Please provide a copy of your final divorce decree.

A court orders the employee to provide health coverage for an eligible child

  • If Mesquite ISD receives a National Medical Support Notice (NMSN) from the Attorney General’s Office, it is mandatory that we enroll your court-ordered dependent in a health plan.  If your court-ordered dependent is not already enrolled in a health plan, that child will be added to the existing health plan that employee is already enrolled in.  If employee is not enrolled in health insurance, employee will have opportunity to enroll in a health plan.  If no election is made, employer reserves the right to enroll employee + child in least expensive health plan available.
  • Effective date of coverage is date that NMSN is received in MISD Benefits Office.
  • Health insurance may not be cancelled and/or terminated without a release form from the Attorney General’s office.

Death of spouse or dependent

The employee involuntarily loses other coverage

(The following reasons do not qualify as special enrollment events:  voluntary termination of coverage such as spouse’s annual enrollment or failure to pay premium; any additional surcharge or benefit reduction for spouse coverage; an increase in the premium cost; a reduction in the employer’s contribution to the premium; any reduction in benefits; or a doctor/provider no longer participates in the plan’s network).

Please provide the Benefits Office with a copy of the Certificate of Creditable Coverage that shows each plan participant that lost health coverage, the effective and end date of coverage.  The effective date of coverage with TRS-ActiveCare (health insurance) will be the first of the following month after the loss of coverage date.  The effective date of other coverage (i.e. life insurance, dental, vision, etc.) will be the first of the month following the date paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).

An eligible dependent involuntarily loses other coverage

(The following reasons do not qualify as special enrollment events: voluntary termination of coverage such as spouse’s annual enrollment or failure to pay premium; any additional surcharge or benefit reduction for spouse coverage; an increase in the premium cost; a reduction in the employer’s contribution to the premium; any reduction in benefits; or a doctor/provider no longer participates in the plan’s network).

Please provide the Benefits Office with a copy of the Certificate of Creditable Coverage that shows each plan participant that lost health coverage, the effective and end date of coverage. The effective date of coverage with TRS-ActiveCare (health insurance) will be the first of the following month after the loss of coverage date. The effective date of other coverage (i.e. life insurance, dental, vision, etc.) will be the first of the month following the date paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline).

The employee and/or dependent(s) obtains coverage elsewhere

You may cancel insurance within 31 days of obtaining new coverage elsewhere. The effective date will be the first of the month following the date paperwork is received in the Benefits Office (as long as paperwork is received within the 31 day deadline). Please provide a copy of enrollment paperwork with effective date of new coverage.

Change in dependent status

  • If a dependent ceases to meet eligibility status for one of the plans, you have 31 days to cancel them from the plan.  Please see benefit summaries and/or plan descriptions for dependent eligibility requirements.

  • The dependent’s insurance will terminate at the end of the month following the month that eligibility ceases; however, the premium will terminate on the first of the month following the date paperwork is received in the Benefits Office.

 

If you experience one of the above changes, you will need to complete paperwork at the Benefits Office within 31 days of the event. If the 31st day is on a non-working day, you need to complete paperwork on a working day prior to the deadline.

To enroll in or change your Benefit Election Form Cafeteria Plan, you will be required to submit documentation to substantiate your request. You may be subject to pre-existing condition benefit limitations.
