Procedure for Enrolling at McDonald Middle School

    Before a student can register, he or she must have the following items:

•  Parent/Guardian must provide updated photo identification
  • Drivers License or Texas ID card
• Withdrawal form and last report card from previous school
  • Must show proof of promotion to the next grade level
• STAAR Results
• Current Immunizations
  • All records must be received before a student is enrolled .  Parents/Guardians are responsible for providing all items. 
  • If necessary, records can be faxed to us at 972-882-5710.
• Original Birth Certificate and Original Social Security Card for the student enrolling
• Proof of Residence
  • We must have documentation showing that the student and his/her legal guardian live within the
  • boundaries for McDonald.  All residency proof will be considered for establishing residency along
  • with home visits from Campus Administration.  Home residency proof must be in the form of the
  • following as all enrollment information will be verified before a student's schedule is completed:
    1. A current utility bill or receipt (stating address) for water, electrical, or gas services to be connected at the address given.  These must be in the name of a parent or guardian.  No cellular telephone bills accepted.
    2. Contract or Mortgage showing ownership of a home at the given address
    3. A current lease for given address: This must be the original lease and it must be on a regulation lease form that has the name of the apartment/home printed on it and listing the student and parent/guardian as a resident.  No generic leases will be accepted.
    4. Updated photo ID with current address
    5. If a parent/guardian or student is moving in with a current MISD resident, they must provide an updated photo ID and two additional forms of residency proof.  ALl registartions requiring special enrollment paparework will need to also prove residency by an Administrative home visit within three to five days of completing the registration process. If renting a house, the landlord's name and phone number must appear on the lease.

    FALSIFYING RESIDENCY DOCUMENTS WILL BE ELIGIBLE FOR A $2,000 FINE.

     

    If any of the above items are missing you will NOT be able to go through the enrollment process.  The campus Principal will verify any questionable residency issues by visiting homes.  Until residency is verified, the student will not be enrolled.
