Enrollment Requirements

  1. Enrolling parent or guardian's picture ID, such as a driver's license.

  2. Proof of residency within the school attendance zone.

     House - Current utility bill with parent name and address or - if renting - a lease

      with parent and child's name listed on the lease.

     Apartment - Lease with parent and child's name on it and a utility bill.

  3. Copy of the child's birth certificate or other document suitable as proof of the child's identity.

  4. Child's Social Security Card

  5. Child's immunization record.

  6. Report Card and/or withdrawal form for your child if previously enrolled.
