We are planning a special day for athletes in Mesquite to obtain their pre-participation physical before the next school year. This will be Saturday May 14th, 2011 from 8:30 a.m. to 1:00 p.m. at Mesquite High School. We are also using the event as a fundraiser for each high school’s athletics department and encourage all athletes to attend.
The cost for each physical is $20. Half of this will be donated back to each participating school’s athletic department. Each athlete will receive a pre-participation physical from a licensed Texas physician.
We will require a parent to fill out the medical history and sign the UIL physical form before a physician can evaluate the athlete.
A copy of the forms and directions can be obtained from the training staff in the school’s athletic department. We are looking forward to a great event and appreciate you letting us take care of your student-athletes.
