New Student Registration
2008 - 2009
Beginning the last week of July, you may call NMHS at 972-882-7900 to schedule an
appointment for registration. A packet will be given to you by NMHS
for you to complete. You will also need the following items
to complete registration. Registration will begin August 12 by appointment only.
| Proof of Residence |
In order to enroll, the student must prove he/she lives in Mesquite. Students under 18 must have their legal guardian's proof of residence. Students 18 or older may have proof of residence in their own name.
- Acceptable Proofs of Residence: Home Owners
- Utility Bill - Currrent Gas, Water, or Electric
- House Deed
- Acceptable Proofs of Residence: Leasing a Home in Mesquite
- Lease agreement with all occupants listed
- Acceptable Proofs of Residence: Apartment Residents
- Apartment Lease with all occupants listed (must have parent and student's name on the lease).

| Withdrawal Papers AND Transcript |

(Including the name, address and phone of previous school) A student cannot be enrolled in two schools at the same time. Students MUST be withdrawn from previous school.

| Immunization Records |
|
Students must have shot records up-to-date before enrolling.

| Special Education Students |
| Need to see a counselor before beginning enrollment process. |
| Birth Certificate or a Copy |
|
| Parent or Guardians Drivers License |
| Address must match proof of residence. |
| Parent or Guardians |
| Must provides all current documentation of legal guardianship. |
