To ALL athletes for the 2007-2008 School Year ~
The following forms need to be completed and turned in before you can participate in athletics for the 2007-2008 school year:
- Physical Evaluation Medical Form (.pdf file)
- Rule Acknowledgment (.pdf file)
- Steroid Agreement (.pdf file)
