Immunization Requirements

 

State law requires that students be immunized against common communicable diseases, including polio, diphtheria-tetanus, pertussis, rubella, measles, and mumps.

Polio    

Three doses of oral vaccine (one of the doses since the fourth birthday)

 

Diphtheria/Tetanus/

Pertussis (DTP/DTaP/DT/Td)              

For students six (6) years or younger - Four doses (one of the doses since the fourth birthday)

DTP     

For every student seven (7) years or older - Three doses (one of the doses since the fourth birthday)

 

DTP Booster              

One dose required every ten years

Rubella *         

One dose of vaccine required on or after the first birthday (German, 3-day measles)

 

Measles *       

First dose of vaccine required on or after the first birthday.

Measles second dose    

CHILDREN BORN ON OR AFTER SEPTEMBER 2, 1991, will be required to show proof of second dose of measles vaccine. CHILDREN BORN PRIOR TO SEPTEMBER 2, 1991, will be required to show proof of second dose of measles vaccine by age 12

 

Mumps *         

One dose of vaccine required on or after the first birthday.

Haemophilus Influenza type b (Hib)     

Children 15 months of age, but not yet five years of age require one dose

 

Hepatitis B *   

Effective August 1, 2000, students born between September 2, 1988, and September 1, 1992 (inclusive) must show proof by 30 days after their 12th birthday of having received 3 doses of hepatitis B vaccine.

 

Children born on or after September 2, 1992, will be required to show proof of three doses of hepatitis B vaccine.

 

Varicella         

Children born on or after September 2, 1994 will be required to show proof of either one dose of varicella vaccine received on or after the first birthday or a parent or physician validated history of varicella illness (chicken pox) or serologic confirmation of varicella immunity.

 

Children born on or after September 2, 1988 but before September 2, 1994,will be required to show proof of either one dose of varicella vaccine received by 30 days past their 12th birthday or a parent or physician validated history of varicella illness (chickenpox) or serologic confirmation of varicella immunity.
