To enroll in Mesquite ISD's Wellness Program, download the registration form below. Completed registration forms should be turned in to your Facility Wellness Representative with your $10 fee or, new this year, you can PAY ONLINE!
For online payment:
- CLICK HERE
- enter your Username (district ID#+LASTNAME)
- select your campus or administration
- select "Wellness"
Wellness forms/membership fees may be turned into your Facility Wellness Representative at any time during the year.
